Fat transfer

  • Fat transfer is the injection of fat to various areas of the body to increase volume. Fat transfer of viable fat cells can augment the treated area, such as the face, breasts, cheeks, lips, buttocks, and chin.
  • Fat transfer is also called fat grafting or fat injection.  The procedure is a complex process: fat must be harvested in such a way as to preserve the integrity of the fat cells. Ordinary liposuction, laser liposuction, and ultrasonic liposuction that disrupts the cell membrane of the fat cell will not produce living fat cells. Concentration of the fat and washing the fat will also affect the results.
  • Results can be long-lasting, but should not be considered permanent. As you age and lose volume in your face, the fat grafting procedure may have to be repeated.
  • Bad results may be the result of a poor fat harvesting technique, causing the fat grafts to be not viable.
  • Fat grafts take better in more highly vascularized area such as the face and do not do as well in highly mobile areas such as the lips.
  • Fat transfer can also help reduce the appearance of acne scars.

Breast lipofilling is a breast surgery technique which involves injecting fat into the breast. It is becoming increasingly popular among women who do not wish to use artificial silicone gel implants.

Indications

Breast lipofilling is a breast enlargement technique which is aimed at women who wish to :

  • Boost the volume of the chest moderately, in order to gain a maximum of one or two cup sizes.
  • Bulge the upper poles of the breasts to enhance the visual effect of the neckline.
  • For those who want a significant increase of volume, the surgeon directly recommends the use of breast implants.The fat can also be used as a filler product to :
    • Conceal the ribs which are too visible beyond the base of the breasts, which cannot have an implant.
    • Correct certain defects caused by implants by avoiding changing them, such as if the implants are visible under the skin or they create a wave effect.

    The fat injection also has a lifting effect on the support of the breasts :

    • Slightly reduce ptosis at the inframammary crease when the breasts are slightly less tense but still firm, in order to gain a bit of tone.
    • Restoring firmness and elasticity to breasts which are less taut.
    • Smoothing a wrinkled neckline, including before the sternum.

    The patient must of course have sufficient reserves of fat. Very thin patients must use an enlargement that involves implants alone or possibly implants combined with lipofilling.

    The latest recommendations from the French High Authority of Health (HAS) no longer set an age limit of 35 years. As a precaution, however, we will avoid using the procedure on women who have a family history of breast cancer or ovarian cancer.

  • How the procedure works

    The procedure is done under a general anaesthetic as an outpatient and lasts between one and two hours.

    As with any lipofilling, the surgeon moves the fat. He starts by performing liposuction where there are sufficient fat reserves, mostly at the level of the saddlebags or stomach.

    He then uses the centrifuge technique on the fat and distributes it evenly in the breasts with fine cannulae. He does not inject more than 200 to 250 cc of fat per breast to maximise the chances of the graft being successful.

    Benefits of breast lipofilling :

    • There is no risk of allergy or rejection because the fat used is from the patient.
    • There are no scars, just very fine incisions hidden under the breast fold.
    • There is no risk of a breast shell.
    • This is a “two in one” operation: the figure is refined by the gentle liposuction and the breasts are reshaped.

    Breast lipofilling and breast implants

    Breast lipofilling is being increasingly associated with the use of breast implants. Indeed, the fat separates the implant from the skin better and provides a more natural look.

    At the end of the procedure, the surgeon closes the incisions again with absorbable sutures and uses a dressing and a compression bra.

    After the procedure

    There is a mild oedema and bruising which resolve in a few weeks. The patient is advised to rest for four to seven days. The patient can play sports again after one month.

    Results

    The result of the breast lipofilling is visible immediately but will become final after three to six months. It is quite normal for the breasts to lose a little volume in the weeks after the procedure. One to three sessions are usually required.

    The appearance of the breasts is very natural. Once the fat graft has taken it will be final. The fatty cells are living tissue. The breasts will also increase or decrease in volume in the event of the patient losing or putting on weight.
